Suggested Steps to Complete PDF Release Forms
These steps can be completed for either version of the PDF forms. Using the printed form these steps will be completed by hand, whereas on the
fillable form you will be able to type the information into the form using your computer. If you would like to hand sign the form you can complete
these steps using either the print or fillable forms.
- Fill in your Legal (Modern) Name.
- Fill in your SCA Name.
- Fill in the section to identify the item being released.
- Creative Work: Check item type(s) and fill in name(s)/title(s) of item. Note: More than one item can be entered
- Limited to what can be legibly listed in the available space.
- Model: Fill in photographer's name.
- Photograph: Fill in name(s) of photograph (for digital image it is suggested to use the file name ex: johnthesmith001.jpg).
Note: More than one photograph name can be entered - Limited to what can be legibly listed in the available space.
- Complete the appropriate grant of use information.
- Creative Work:
- Determine if Option #1 applies. - If yes, check the box and SKIP past other options section.
- Complete Other Options section as applies - Note: Only complete if box in Option #1 above is NOT checked.
- Determine if you agree or do not agree to allowing editing or abridgement. - Uncheck the box that does not apply
(ex: Uncheck do not agree if you will allow editing).
- Model:
- Determine if Option #1 applies. - If yes, check the box and SKIP past other options section.
- Complete Other Options section as applies - Note: Only complete if box in Option #1 above is NOT checked.
- Photograph:
- Complete Grants Of Use section. - Check applicable boxes and fill in information as it applies.
- Determine if Perpetual Grants Of Use applies. If yes, check the box. - Note: If the box is checked in this step,
all boxes and information in the Grants Of Use section above will not apply.
- Read the information above the signature area.
- Fill in date, email address and mailing address - If you are completing the model release as a parent or guardian, also fill in
your name and date as applicable
- Sign and submit the completed form. - See the section below that best fits how you will be signing the form.
Steps to Digitally Sign and Submit PDF Form
NOTE: Once this section is completed you will not be able to change the information that you entered in the PDF form. Please make
sure all fields are completed the way that you intended before continuing!
Before going further you will need to make sure that your computer gives you the option to digitally sign the form. Please check to
see if you have an orange bar in the signature box as the sample images shows below:

If you do not see the orange bar, your computer may need the Adobe software to be updated. Please visit
http://get.adobe.com/reader/ to update your Adobe software.
If you do see the orange bar please follow the steps below:
- Click the orange bar.
- Select a new digital ID... (As shown below - We will work on the basis that you need to create a digital signature).
Then click the Next> button.

- Select New PKCS#12 ID file (As shown below - The other type may be choosen but these steps will follow selection of PKCS).
Then click the Next> button.

- Fill in your Name and Email Address (As shown below - This is a legal signature, please do not use your SCA name).
Then click the Next> button.

- Fill in the Password and Confirm Password sections (As shown below).
Then click the Finish button.

- Check the Lock Document After Signing box (As shown below).

- Fill in the Password box (As shown below). Then click the Sign button. (The file will be saved as a signed PDF file.
No changes will be allowed to the form after this step).

Once the form has been digitally signed the signature box will be similar to the sample below:

- Submit the form by attaching the PDF file to an email addressed to the Deputy Webwright in charge of releases at
and the Chronicler / Webwright / Wiki Administrator of the
respective publication / website that is publishing your submission.
Steps to Hand Sign and Submit PDF Form
- Sign the form in the Signature box.
- Scan the form as a PDF file (Note: The file can be saved as a standard PDF or searchable PDF so long as it can
not be editted once scanned).
- Submit the form by attaching the PDF file to an email addressed to the Deputy Webwright in charge of releases at
and the Chronicler / Webwright / Wiki Administrator of the
respective publication / website that is publishing your submission.
Note: If you are unable to scan the form into a PDF file format please contact the Deputy Webwright in charge of releases at
for alternate ways to submit the form such as postal mail, fax or hand
delivery at an event or meeting.
